Because Together, We Can Achieve More for Our Communities! |
Dear Friends, Partners, and Supporters, This Giving Tuesday, I’ve been reflecting deeply on what it means to truly show up for community, especially in a year marked by painful funding cuts. At Health Outreach Partners, our mission has always been to advance health equity for those too often left at the margins: agricultural workers, rural families, immigrant communities, and people with intellectual and developmental disabilities (IDD). That commitment is stronger than ever, but several core funding streams have been reduced or eliminated at the very moment our communities need us most. Even in this challenging landscape, HOP’s impact remains powerful and essential: - Strengthening Outreach & Enabling Services: Training and supporting health centers to integrate outreach and improve access to care. In just the past year, we have trained more than 4,300 health center staff across the country.
- Transportation Access: Helping clinics reduce missed appointments through our Transportation Toolkit 2.0 and our Return on Investment tools. In the last year, we facilitated more than a dozen activities and published 4 resources to assist communities in accessing healthcare and measuring the financial impact of outreach programs.
- Agricultural Worker Health: Supporting providers to address pesticide exposure, extreme heat, maternal health risks, and environmental health issues.
- Inclusive Health for People with IDD: Co-creating tools and e-learning grounded in “Nothing for us, without us,” ensuring truly inclusive care, serving over 500 people in our training series.

| None of this work happens in isolation. It is built through deep relationships—with outreach workers, community health workers, promotores, regional centers, farmworker-serving organizations, families, and people with lived experience. It is guided by principles of inclusion, ensuring the solutions we create are shaped by the very communities most impacted by inequities. We invite and include community members to participate in our program design and facilitation. Even in a year of budget cuts and uncertainty, these partnerships have been a constant source of hope and inspiration. And this is why your support matters so deeply this Giving Tuesday. Your generosity helps sustain programs at risk of being scaled back, keeps our tools and trainings free for safety-net providers, and ensures that communities who already face barriers to care aren’t left behind because of funding cuts. How you can help today: - Make a one-time or monthly gift
- Ask your employer about matching donations
- Share our campaign with your networks
