The Inclusion Scorecard for Population Health™ (ISPH) is an online, web enabled assessment tool featuring over 80 emerging and best practices that advance health equity. These have been sourced from publications by the CDC, American Hospital Association, HRSA’s Health Center Program Compliance Manual and the American Medical Association and cross referenced with the national CLAS standards linked to serving diverse patient populations.
The best practices or activities are divided into four focus areas as follows:
- Tracking the Metrics of Inclusion for Healthcare
- Creating a Culture of Inclusion to Support Health Equity
- Managing the Link Between Equity and Quality of Care
- Inclusive Community Engagement
The Scorecard has an integrated knowledge base that can support leaders and staff who are interested in adopting these best practices. Resources include reference material, examples of other systems use of the practice and key implementations strategies.
