| Screening for SDOH Learning Collaborative Announcement Please join the Farmworker Health Network for our Screening for Social Determinants of Health (SDOH) Learning Collaborative (LC)! This LC will build upon year one to integrate health center best practices on screening for and addressing SDOH factors that impact the Migratory and Seasonal Agricultural Worker (MSAW) population. Health center staff recommended to participate in this LC include: clinical supervisors and coordinators, medical assistance, patient navigators, community health workers, and front office workers. CNE credits will be available. |

SDOH Community Partnerships LC (Year 2) Learning Collaborative Announcement Addressing the social determinants of health to improve health outcomes requires successful, multi-sectoral partnerships. Health Outreach Partners, Farmworker Justice, and the National Center for Farmworker Health invite health center staff to join Year 2 of a 3-year learning collaborative. This year’s learning collaborative will consist of 4 sessions that will focus on exploring and developing effective strategies for building and re-establishing community partnerships that support and provide solutions to address social determinants of health of Migratory and Seasonal Agricultural Workers (MSAWs). The Learning Collaborative will meet on a bi-weekly basis beginning Wednesday, January 12, 2022 at 10AM PST/1PM EST. All applications must be received by Friday, January 7th at midnight local time. 2022 SDOH Academy Innovations Showcase Competition Activity Announcement The Social Determinants of Health (SDOH) Academy invites you to participate in the 2022 SDOH Academy Innovations Showcase Competition: Helping Health Centers Build Healthier Communities. This interactive training opportunity will allow health centers to learn how other multidisciplinary health care teams apply and share solutions to unmet needs while addressing SDOH. Participants will be able to: - Attend a live kick-off webinar featuring dynamic speakers from the SDOH field.
- View three on-demand (pre-recorded) micro-training modules and receive a certificate of completion to document their foundational knowledge of the SDOHs and Healthy People 2030 goals.
- Enter an abstract to compete for an opportunity to be featured on a live national audience webinar showcasing their innovations, “Helping Health Centers Build Healthier Communities.”

In it, we describe the importance of screening for structural vulnerability in order to provide your patients with compassionate, comprehensive care that takes into consideration the structural factors that impact their health and wellbeing outside the clinic. HOP describes what the Structural Vulnerability Assessment Tool is and how it can be used to screen for structural factors that can impact the effectiveness of care. Welcome Yuriko de la Cruz! New Board Member Announcement Yuriko de la Cruz, MPH, CPHQ (she/her/ella) is the Social Determinants of Health Manager at the National Association of Community Health Centers. She currently partners with health centers to identify patient-level social needs and to use SDOH data (specifically PRAPARE) to inform upstream community efforts that advances health equity. Yuriko started her career as a Peace Corps volunteer in the Dominican Republic. After her service, she worked as a Community Health Worker in Newark, NJ, first at a community-based organization and later at a hospital system. Yuriko later worked in training, technical assistance, and grant writing for HIV/AIDS prevention, substance use, and cultural competency programs on a national and statewide level. She then pivoted into healthcare and focused on population health, practice transformation, and continuous quality improvement within primary care settings and federally qualified health centers. Welcome Yuriko! |

| Help Shape a Healthier California Coffee Break Webinar In a recent study, California was ranked #2 among all states in racial/ethnic diversity and #1 in linguistic diversity. The pandemic amplified the vulnerability of many racially and ethnically diverse communities in the state and their need for trusted messengers who could support education, outreach, and recovery. Community Health Workers (CHWs) play an integral role in ensuring a healthy California. CHWs have always been trusted members of their communities but are especially visible now as part of the public health response during times of crisis. In an effort to build a multicultural support network for CHWs of all job titles and identities, key stakeholders, CHWs and CHW allies from throughout California have been meeting since January 2021 to form a Community of Practice to work toward the common goal of supporting the role of all CHWs and amplify their voice in achieving health equity and social justice. |
