Health Outreach Partners, in partnership with Special Olympics, is proud to share, Pathways to Inclusion: Advancing Health Access for the IDD Community — a 16-video asynchronous training series designed to strengthen inclusive, person-centered care across the health system.

We’re excited to share that the first five videos in our Social Determinants of Health module are now live on our website! These sessions explore the real-world barriers people with intellectual and developmental disabilities face—and offer practical strategies health centers can use to better support access, engagement, and outcomes.
